Refactor ssl check to mimic Rack::Request#ssl? behavior as stated in #663 . I've removed the query assigment since it looked redundant just after the gsub and added a few test for a custom full_host that were not provided before.

Improves error handling behavior. Closes #578 .
...
* OmniAuth::FailureEndpoint will raise any encountered
errors out if RACK_ENV is development.
* It will redirect compatibly with current OmniAuth if
not in development.
* Now uses Rack::Response to include a content length
in the failure response.
