Merge branch 'master-security'

* master-security:
  Ensure [] respects the status of the buffer.
  delete vulnerable AS::SafeBuffer#[]
  use AS::SafeBuffer#clone_empty for flushing the output_buffer
  add AS::SafeBuffer#clone_empty
  fix output safety issue with select options
