e828657973 Move transaction thread-safety test to transactions_test. Check that simultaneous transactions don't step on each others' toes. Check that simultaneous transactions don't give dirty reads (read-committed txn isolation or greater.)

2c0fa32088 Do not use instantiated fixtures (in general) in tests. Also, support the use of transactional fixtures by setting the AR_TX_FIXTURES environment variable to "yes".

44819b4717 Fixed that nested transactions now work by letting the outer most transaction have the responsibilty of starting and rolling back the transaction. If any of the inner transactions swallow the exception raised, though, the transaction will not be rolled back. So always let the transaction bubble up even when you've dealt with local issues. Closes #231 and #340.
