parse.y: ripper_lex_state_name

* parse.y (ripper_lex_state_name): represent lex_state as OR-ed
  form.

* ext/ripper/lib/ripper/lexer.rb (Ripper::Lexer::Elem#to_a):
  lex_state for inspection.
