akr eeb5c7e18e * ext/dbm/dbm.c (fdbm_initialize): accept optional 3rd argument to
specify an open flag.
  (Init_dbm): define open flags: DBM::READER, DBM::WRITER, DBM::WRCREAT
  and DBM::NEWDB.
