Nobuyoshi Nakada c28a4e0340
Replace "Fixes"/"Fixed" in commit logs as well as vcs.rb [ci skip]
Use the same regexp to replace "(#NNNN)" and "GH-NNNN" style
references in vcs.rb, too.
