Jan Provaznik 0fc9f9d3e7 Add version 4.2 to all existing migrations 
DB schema generated by a migration may look different in
rails 4 and 5 (because rails 5 may use different default values).
For this reason it's important to explicitly set for which rails
version a migration was written for.

Stan Hu 1e97a6df24 Add index to labels for `type` and project_id` 
When loading pages that display the number of open issues,
the backend runs a query such as:

```sql
SELECT "labels"."id" FROM "labels" WHERE "labels"."type" IN ('ProjectLabel') AND "labels"."project_id" = 1000
```

This results in an entire scan of the `labels` table. To optimize performance,
add the appropriate index to the table.
